One record, twelve systems: what a patient journey really touches

Ask a hospital how many systems a patient touches between walking in and going home, and the honest answer is usually a shrug. Not because nobody knows, but because nobody has ever had cause to count. Here is what happens when you do.

Follow one visit all the way through

A patient arrives at the front desk. Their identity is captured possibly for the first time, possibly for the fourteenth. They are registered against a number that means something to this hospital and often nothing anywhere else. That is system one.

They are triaged, seen, and something is ordered: bloods, an image, a swab. The order leaves the clinical system and arrives in the laboratory or radiology system, which has its own idea of who this patient is and its own way of describing the test. That is systems two and three, and the first place where a person is quietly re-identified by hand.

The result comes back. It lands somewhere. If the join between the ordering system and the resulting system is good, it lands against the right patient in the right encounter. If it is not, it lands in a worklist that somebody reconciles later, which is a polite way of saying somebody retypes it.

Then pharmacy. Then the ward, with its own charting. Then billing, which needs to know what was actually done rather than what was ordered. Then discharge, which needs to summarise all of it for somebody outside the building who has access to none of it. That is easily a dozen systems, and we have not left the hospital yet.

The gaps are not where you would expect

The failure is rarely a system being down. Systems are mostly up. The failure is at the joins, and the joins are usually staffed by people rather than software.

Somebody reads a number off one screen and types it into another. Somebody notices that the patient in the lab system has a slightly different date of birth and makes a judgement call. Somebody keeps a paper list because the handover screen does not show the three things they actually need together.

None of that is anybody's fault. It is what happens when a journey is continuous but the software underneath it is not. Every manual join is a place where the record can diverge, and the cost of divergence is paid much later, usually by somebody who was not there when it happened.

What 'one record' has to mean

'One record' is easy to say and easy to fake. A dashboard that reads from six systems and shows them side by side is not one record. It is six records in a shared frame, and the moment two of them disagree the frame is worse than useless, because it looks authoritative.

The version worth building means something stricter: one identity that every part of the journey resolves to, one timeline that events attach to rather than sit beside, and one place where the question 'what actually happened' has an answer that does not depend on who you ask.

That is the shape Zypocare One is built around. Access, care, operations, billing, discharge and follow-up are not separate products with a bridge between them they write to the same record because they are the same system.

Design for the join, not the screen

Most software in this domain is designed screen-first: what does the clinician see, what does the clerk see, what does the biller see. That produces good screens and bad joins, because the join is nobody's screen.

We try to work the other way round. Before designing a view, we ask what has to be true for it to be trustworthy: which identity resolves it, which events feed it, what happens when one of those events arrives late, out of order, or twice.

Late and duplicate events are the ordinary case, not the exception. A result can arrive after a discharge summary has been written. An amendment can arrive after a bill has been raised. If the design assumes tidy ordering, it will be wrong in production within the first week.

What changes day to day

The visible change is unglamorous: fewer places where somebody has to retype something, and fewer moments where two screens disagree and a person has to decide which one to believe.

The less visible change matters more. When the record is continuous, questions that used to require an investigation become ordinary lookups. When was this ordered, by whom, and what happened to it. Who saw this result and when. What did the patient actually consent to, and is that still current.

None of those questions are exotic. They are the questions people ask every day and currently answer by walking somewhere.

What this does not solve

It does not make the systems you are keeping disappear. Most hospitals run software they have good reasons not to replace, and a platform that assumes otherwise is a platform that never goes live. Connecting to those systems properly is its own discipline, which is why the Integration Platform is a first-class part of the product rather than a professional-services engagement.

It does not fix a process that was already broken on paper. If nobody owns a step today, software will not invent an owner for it. It will, at best, make the absence obvious.

And it does not remove the need for the people at the joins. It removes the need for them to be the join.
